Another Formula for Chancre

Ilbang

한글 일방(삼부약)  ·  Pinyin Yi Fang

一方

The head of this leaf is literally 一方, an unnamed alternative formula, so it is entered under an identifier taken from the section it stands in: it is the first unnamed leaf of the 糝付藥 subsection, the dusting-and-dressing powders of 陰蝕瘡, at 048a. It has nothing to do with the three unnamed 一方 entries of the 黃疸 chapter (chihyeok-bang-1 and following), with the 肺癰 one of 癰疽門 (pyeongong-ilbang), or with the 疔疽 one (jeongjeo-ilbang). The two 又方 leaves that follow it in the same subsection are entered as sambuyak-ubang-1 and sambuyak-ubang-2. The transcription writes 鈆 for 鉛 and the corpus prints 鈆 in the majority of places, so it is kept; the drug is entered under 黑鉛. 鵬砂 is the transcription's form for 硼砂, borax, and the corpus prints 鵬砂 in seventeen places, so it is kept. 幷 is left as the transcription prints it.

Translation

Treats chancre, and the penis wholly eaten away. Use nothing but this medicine and it will grow out again as it was at first, only the glans will be smaller. Black lead 5 don, melted down; cast in quicksilver 2 don 5 fun and grind until no beads show; add glauberite 3 don 5 fun, calomel 2 don 5 fun and borax 1 don. Grind the above to an extremely fine powder. First wash the affected place with a decoction of scallion, mugwort and Sichuan pepper, then dust the powder on dry. It also treats a tongue bitten off by another, with an effect like magic. — Hoechun 回春 (Donguibogam · Jechang)

Clinical commentary 臨床

Half a don of lead melted in a crucible, two and a half don of quicksilver stirred into the melt and ground until the mercury no longer shows as separate beads, then glauberite, calomel and borax ground in. 硏不見星 — grind until no stars are seen — is a real amalgam-making instruction, and what comes out is a lead-mercury amalgam powder. The indication is the most extreme in the section: 玉莖蝕了, the penis eaten away, with the promise that it will grow back as before and only the glans be smaller. That cannot happen, and the claim is worth stating plainly rather than translating past. The appended line about a tongue bitten through belongs to the same class of claim. The preliminary wash of scallion, mugwort and Sichuan pepper is the one defensible part. **The powder is a lead and mercury amalgam with added calomel, dusted onto a destroyed and raw genital surface. Both metals are absorbed through ulcerated skin and accumulate; the exposure here is large and the surface is the most absorptive possible. Lead causes nephropathy, anaemia and neuropathy, and mercury the ulcerating, suppurating eruption the chapter itself records at 2.2 as a complication of mercury treatment. This must not be prepared.** Destructive genital ulceration is an emergency needing cultures, serology and systemic antibiotics. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
