Another Formula for the Forced-Emission Sore

Ubang

한글 우방(삼부약 1)  ·  Pinyin You Fang

又方

The head of this leaf is literally 又方, so it is entered under an identifier taken from its section: it is the first of the two unnamed 又方 leaves of the 糝付藥 subsection of 陰蝕瘡 at 048a, the one that follows the 一方 entered as sambuyak-ilbang. It is unrelated to the other ilbang and ubang entries of the corpus. The transcription reads 畏疼不敢泄刮, which does not parse cleanly; 洗刮, dares not wash and scrape, would be expected, and the transcription's reading is printed with the difficulty noted. The final clause is punctuated 勿令陽道興起脹斷瘡靨, 靨堅自安矣, taking 瘡靨 as the object of 脹斷. No weights are given anywhere in the leaf, so no gram figures are entered.

Translation

Treats the case of a young man whose yang way rises strongly, who does not emit when he should and then forces the emission when he has not; the swelling tears the tender skin. At first it is like the eye of a needle, and for fear of the pain he dares not clear and scrape it, so that in time the ulceration runs along the whole shaft and the pain grows worse daily. Use a decoction of schizonepeta, phellodendron, licorice, fresh scallion and lycium root bark, and wash the crusted pus away; then burn chebula to ash, add a little musk, and dust it dry on the affected place. He is also to break off intercourse and to drink cold water at bedtime, so that the yang way does not rise and tear the scab of the sore open again; once the scab is firm it settles of itself. — Deukhyo 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

This is the best clinical writing in the section and it is not really a prescription. A young man with strong desire holds back emission and then forces it; the engorged shaft splits the thin skin; the tear starts as a pinhole, he will not touch it because it hurts, and weeks later the whole shaft is ulcerated. Every step of that is a plausible sequence — a frenular or preputial tear, neglected, becoming secondarily infected — and the leaf has watched it happen. The treatment follows the observation. Wash the crusts off with a bland decoction, dust the raw surface with charred chebula, which is heavily tannic and will astringe a weeping tear, and then the two instructions that actually matter: stop intercourse, and do not let an erection pull the scab open again. 靨堅自安矣 — once the scab is firm it settles of itself — is a correct account of how such a tear heals, and the cold water at bedtime is a crude but honest attempt at the hardest part of the regimen, which is mechanical rest. **Musk is taken from a CITES-protected species and must not be used; charred chebula alone would serve the same astringent purpose.** A tear on the shaft or frenulum that has ulcerated needs examination and swabbing rather than self-treatment, and if it recurs the foreskin itself may be the problem. But the advice to rest it and let the scab set is sound and has not changed. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
