Translation

Treats a man who, after taking a judicial flogging, has a hard eschar over rotting flesh with pain beyond bearing, so that he cannot rise or move. Take this and the pain stops and he can walk at once — its effect is like a spirit's. Four nyang of tree ear, put into a sand pot and dry-fried until charred with its nature retained, then powdered. Take 5 don of the above to a dose, stirred into a bowl of hot wine. A little while after taking it the force of the drug travels up to the flogging wound and pierces out from inside the flesh like a needle; the itching is severe and blood-water runs from time to time. Wash it clean at once with medicinal water and stick a plaster on. — Manbyeong Hoechun 回春 ○ To take the eschar off: take egg white with a little musk put into it, whip it to a thin water with a silver pin, and dab it on lightly with the point of the pin. In no long time the eschar turns soft and rotten and can be taken away; change it once a day and stick a plaster on. When the dead flesh has all been dissolved, in a few days it is as it was. — Manbyeong Hoechun 回春 (Donguibogam · Jesang)

Clinical commentary 臨床

One drug under a grand name: four nyang of tree ear charred black in a sand pot — hence 烏龍, the black dragon — powdered and taken five don at a time in a bowl of hot wine. The complaint is specific and belongs to the 杖傷 section at its ugliest: a man some days after a judicial beating, with a hard black eschar over rotting flesh, in too much pain to stand. The leaf then promises what the patient will feel, a piercing like needles from inside the flesh, itching, and blood-water running from the wound. The second half of the leaf is the part worth keeping. Egg white with a little musk, whipped thin and dabbed on the eschar daily until it softens and lifts away, is debridement — the removal of dead tissue so that living tissue can granulate underneath. That is still the correct manoeuvre for an eschar over a contused wound, and the chapter arrives at it by the wrong reasoning. The charred fungus taken in hot wine does nothing of the kind; what it would produce is the warmth and flush of the wine, which the text reads as the drug's force arriving at the wound. **Raw egg white laid on an open wound is a culture medium, and this is a dirty wound days old: the infection and tetanus risk is real. The formula also contains musk (麝香), from a CITES-protected animal. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
