Supplement Qi and Engender Blood Decoction

Bogisaenghyeol-tang

한글 보기생혈탕  ·  Pinyin Bu Qi Sheng Xue Tang

補氣生血湯

白朮 and 熟地黃 are printed with the standard glyphs for the transcription's 白术 and 熟地黄, and 《醫鑑》 for 醫鍳. The 杖傷 section's own prose list writes this formula's name 補氣坐血湯, with a collation note at the foot of the 化瘀散 leaf giving 生 as the First Edition's reading; the leaf head and the leaf itself both read 補氣生血湯.

Translation

Treats a flogging wound that has ulcerated and festered and has long failed to heal. Ginseng, white atractylodes, white poria, white peony, Korean angelica, tangerine peel, cyperus, fritillaria, platycodon, prepared rehmannia and licorice, 1 don each. Cut the above, make one packet, and decoct in half wine and half water and take. — Gogeum Uigam 醫鑑 (Donguibogam · Jesang)

Clinical commentary 臨床

A tonic base with a few additions, and the one formula in the 杖傷 section aimed at the aftermath rather than the injury. Underneath it is 八物湯 — the four Qi drugs of 四君子湯 (ginseng, white atractylodes, white poria, licorice) with the Blood drugs Korean angelica, white peony and prepared rehmannia, cnidium alone left out — and over that tangerine peel and cyperus to keep the Qi moving so a heavy tonic does not stagnate, with fritillaria and platycodon to open and discharge what is still suppurating. Half wine and half water, as the chapter does for wounds. The instinct is sound, and worth saying so: faced with a wound that will not close, the formula supports the patient instead of attacking the sore. A flogging wound in a prisoner that has been ulcerated for weeks is a problem of nutrition, infection and dead tissue, and what closes it is debridement, dressing, antibiotics where there is cellulitis or osteomyelitis, and enough protein and calories to build granulation tissue. A ginseng-and-rehmannia decoction is not a substitute for any of that, though in a starved patient the intention behind it was pointing the right way. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
