Donguibogam 東醫寶鑑

治積聚癥癖, 黃疸, 鼓脹, 十種水氣, 八種痞塞, 五種淋疾, 九種心痛, 遠年瘧疾, 及療七十二種風, 三十六種尸疰, 癲狂, 邪祟, 一切腹中諸疾. 川烏(炮) 二兩半, 吳茱萸ㆍ桔梗ㆍ柴胡ㆍ菖蒲ㆍ紫菀ㆍ黃連ㆍ乾薑(炮)ㆍ肉桂ㆍ川椒(炒)ㆍ巴豆霜ㆍ赤茯苓ㆍ皂莢(灸)ㆍ厚朴ㆍ人參 各五錢. 右爲末, 煉蜜和丸梧子大. 薑湯下三丸, 或五丸至七丸. 《局方》 一名萬病紫菀丸. 《海藏》 治婦人腹中積聚, 有似懷孕, 羸瘦困弊, 或歌哭如邪祟, 服此藥自愈, 久病服之, 則皆瀉出蟲蛇惡膿之物, 乃愈. 《得效》

Translation

Treats accumulation, aggregation, concretion and aggregation; jaundice; drum distension; the ten kinds of water qi; the eight kinds of blockage; the five kinds of strangury; the nine kinds of heart pain; malaria of long standing; and it cures the seventy-two kinds of wind, the thirty-six kinds of corpse infixation, mania and withdrawal, spectral influence, and all the various diseases within the abdomen. Aconite (blast-fried) 2.5 nyang; evodia, platycodon, bupleurum, acorus, aster, coptis, dry ginger (blast-fried), cinnamon bark, zanthoxylum (stir-fried), croton frost, red poria, gleditsia pod (mix-fried), magnolia bark and ginseng 5 don each. Powder them, make honey pills the size of a firmiana seed, and take three down with ginger decoction, or five to seven. Also called Manbyeong Jawon-hwan. It treats accumulation in a woman's abdomen resembling pregnancy, with emaciation and exhaustion, or singing and weeping as though under spectral influence; taking this medicine, she recovers of herself. In long-standing disease, worms, snakes and foul pus are all purged out, and then recovery follows. — Hwaje Gukbang 局方 (Donguibogam · Jeokchwi)

Clinical commentary 臨床

The indication list is the longest in the Bogam and should be read as a claim about scope rather than a differential. What holds it together is the mechanism: two and a half nyang of blast-fried aconite with croton frost behind it, a warm attack on cold binding, and everything else restraining or directing it. The Deukhyo passage appended here is the same picture the Phlegm-Rheum chapter treats at 6.11 — a woman singing and weeping as though possessed — and again the answer is that it is not possession. That two chapters reach the same conclusion by different routes is worth noting. **Contains aconite at high dose with croton frost, both toxic.
