Clinical commentary 臨床

The counterpart to the purging formulas, and the chapter is explicit about which patient gets which: 腹皮急, 按之痛 with heat is the hot bound pattern and takes rhubarb, while 腹皮急, 按之濡 with no fever is 陰冷所成, made by yin cold, and takes this. Ginseng and astragalus hold the centre, cinnamon bark and costus root warm and move, Korean angelica with cnidium and peach kernel move the blood, and moutan bark with coix seed stay in the formula to cool and drain what suppuration there is. It is an 內托 formula in all but name. 時下膿血, pus and blood coming away from time to time, is the sign that the abscess has already opened into the bowel. A patient like that is not acutely septic and not a candidate for a purge; what the tradition offers instead is support, and the reading is a chronic walled-off collection or a fistula draining into the gut rather than an acute appendicitis. The gastrodia is the odd drug here, and the Immun gives no reason for it. **A chronic abdominal abscess or a fistula needs imaging and, usually, drainage or surgery. Supporting the patient is not the same as resolving the collection.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
