Patrinia Powder

Paejang-san

한글 패장산  ·  Pinyin Bai Jiang San

敗醬散

The transcription prints 癕 for 癰; 癰 is printed above. 敗醬, patrinia (Patrinia scabiosifolia), the 1 don 5 fun middle drug of the three and the one the formula is named for, had no entry in herbs.yaml and has been registered as paejang from its own 湯液篇 草部 head (tang3 007a, 主破多年凝血, 能化膿爲水 … 又排膿補瘻). The second name given, 薏苡附子敗醬散, is the Geumgwe Yoryak formula from which this leaf descends.

Translation

Treats intestinal abscess with no fever in the body and a soft, cold abdomen. Coix seed, 2 don 5 fun; patrinia, 1 don 5 fun; blast-fried aconite, 5 fun. Cut the above, make one packet, decoct in water and take. Pus and blood should come down with the urine, and the patient recovers. — Immun 入門. Also called Uii-buja-paejang-san, the Coix, Aconite and Patrinia Powder. — Junggyeong 仲景 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Three drugs, and the Geumgwe formula under a shortened name. Coix seed drains, patrinia cools and breaks the pus, and a small amount of blast-fried aconite warms the cold in which the whole thing is sitting. The ratio matters: the aconite is 5 fun against 2 don 5 fun of coix, a warming hint rather than a warming formula, and the Geumgwe's own instruction is the same. The indication is the chapter's cold abscess — 身無熱, 腹濡冷, no fever and a soft cold belly — which is the chronic, walled-off collection rather than the hot acute one, and 小便當下膿血 reads as the collection discharging into the bladder or simply as the tradition's way of saying the pus must find an exit. Patrinia has stayed in East Asian practice as an antibacterial drug for suppuration, and this is the formula where it carries the name. **Contains aconite, which is toxic. This must not be prepared. An intra-abdominal abscess needs imaging and drainage, and the absence of fever does not make it safe to treat with a decoction.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
