Red Atractylodes Formula

Jeokchul-won

한글 적출원  ·  Pinyin Chi Zhu Yuan

赤朮元

赤朮 is the old name for the red grade of 朮, which is 蒼朮, and it is entered here as changchul; the name is not separately registered in the herb list, and the processing the leaf prescribes — steeping in rice-washing water and drying over a fire — is the standard preparation of 蒼朮, which confirms the identification. 川練子 is the corpus form for 川楝子 and 破故紙 for 補骨脂. The Sichuan pepper and the scallion white are boiled with the atractylodes as part of its processing rather than taken as drugs of the formula, and are entered without weights. The honey that binds the pills is kept in the source text only. The collation note 䓗 : 葱 at the page boundary of leaf 005b annotates this leaf's 䓗白, the only one on the page; the corpus form 葱 is printed, which is also the first edition's reading.

Translation

Treats a deep-rooted bone abscess long failing to heal. Red atractylodes 1 geun, steeped in rice-washing water to take out the oil, then boiled with Sichuan pepper and scallion white until black and dried over a fire; fennel fruit, psoralea, toosendan fruit stir-fried, red poria, angelica dahurica and peach kernel, 1 nyang each. Powder the above and make honey pills the size of a firmiana seed; a hundred pills to a dose, sent down with warm water. — Deukhyo 得效 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Sixteen nyang of one drug against one nyang each of six others — the formula is atractylodes lancea and a short tail. The processing is the interesting part: the drug is steeped in rice-washing water to draw off its oil, then boiled with Sichuan pepper and scallion white until it blackens and dried over a fire, which is a warming and drying preparation laid on top of a drug that is already the chapter's chief damp-drier. The tail is not a sores set at all. Fennel fruit, psoralea and toosendan fruit are the warming lower-body group that the Donguibogam uses for cold lodged below the waist, red poria drains, angelica dahurica is the one drug pointed at the sore, and peach kernel breaks the Blood. Read together with the indication — a bone abscess that has gone on for a long time — this is a formula for the cold, indolent, chronic end of 附骨疽 rather than for an acute one, and it is the counterpart of the heat-draining decoctions earlier in the section. A hundred pills to a dose of a formula that is four-fifths atractylodes is a large and sustained dose. Chronic osteomyelitis does not resolve on a warming interior formula; it needs imaging, cultures, prolonged antibiotics and removal of the dead bone. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
