Green Peel, Licorice, Atractylodes and Phellodendron Decoction

Cheongchochangbaek-tang

한글 청초창백탕  ·  Pinyin Qing Cao Cang Bai Tang

靑草蒼栢湯

栢 is left as the transcription and the corpus both print it, and 條芩 is the corpus form for the slender-root grade of 黃芩. The 始發預防法 passage standing earlier on the same leaf names this decoction and adds that if the pain at the 環跳 point does not resolve, 麻黃 1 don is added and two or three packets given; that addition belongs to the passage and not to this leaf, and is not entered in the ingredients. The collation note 䓗 : 葱 at the page boundary below this leaf annotates the 䓗白 of the 赤朮元 leaf above, not anything in this one.

Translation

Treats a deep-rooted bone abscess just beginning, and should be given as a preventive. Green tangerine peel 1 don 5 fun; licorice node 5 fun; atractylodes lancea and phellodendron, 3 don each. Cut the above, make one packet, decoct in water, stir in three spoonfuls of ginger juice, and drink it on an empty stomach. In winter add cinnamon twig; in summer add slender scutellaria; for a depleted body add achyranthes. — Immun 入門 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Four drugs, and the name simply lists them. At its centre is 二妙散 — atractylodes lancea and phellodendron, the Donguibogam's standing pair for damp heat pouring downward — given here at 3 don each, which is the full dose of that formula rather than a token of it. Green tangerine peel moves Qi in the flanks and hip, the licorice node is the part of the root the sores tradition reserves for suppuration, and three spoonfuls of ginger juice are stirred into the finished decoction. What makes the leaf unusual is that it is explicitly preventive: 始作, 宜預防 — just beginning, and it should be used to forestall. The 始發預防法 passage a few lines above says the same thing from the other end, that unremitting pain at the 環跳 point in the hip is the warning of a coming 附骨疽 and that this decoction should be given at once, with ephedra added and the patient finally sat over a heated pit if it fails. Reading hip pain as the prodrome of a deep bone infection is a genuinely sharp observation; acute haematogenous osteomyelitis and septic arthritis of the hip do begin as unrelenting pain over the joint before anything is visible. The seasonal modifications are the chapter's habit rather than this formula's logic — cinnamon twig for winter cold, slender scutellaria for summer heat, achyranthes to lead the formula down the leg in a weak patient. Suspected hip or bone infection is an emergency and needs urgent imaging, blood cultures and joint aspiration, not a preventive decoction. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
