Pinch of Gold Powder for Goose-mouth Sore

Illyeomgeum-san

한글 일념금산 (소아)  ·  Pinyin Yi Nian Jin San

一捻金散

治同上 points back through 朱礬散 to 保命散 and so to 鵝口瘡. Donguibogam carries a second 一捻金散, the Uihak Jeongjeon formula of the Genitals chapter for mounting qi (corydalis, toosendan, scorpion and fennel), registered as `illyeomgeum-san`; the transcription writes that leaf head 一捏金散 and `leaf-names.yaml` pairs the two spellings. The two formulas share nothing but the name. A bare 一捻金 of the Abscesses chapter is registered as `illyeomgeum`, and 一捻金元 of the Genitals chapter as `illyeomgeum-won`.

Translation

Treats the same as above. Realgar 3 don, borax 1 don, licorice half a don, a little borneol. Grind to a powder and dust it on dry, or mix it with honey and paint it on. — Dansim 丹心 (Donguibogam · Soa)

Clinical commentary 臨床

Borax and borneol are the working part of this — borax is still the standard folk remedy for thrush in much of the world, and it does have some antifungal action — with licorice to soothe and realgar, again, for the colour and the tradition's reading of orpiment and realgar as toxin-killers. Of the three goose-mouth powders in the section this is the only one that is not built on mercury, and the only one that offers a dry dusting as the first option. That does not make it safe. **Contains realgar, which is arsenic sulfide, at three don — the largest ingredient — put into the mouth of a newborn who will swallow it, with no dose by weight given anywhere in the chapter. Arsenic in an infant is absorbed across mucosa, cleared poorly by immature kidneys, and injures the nervous system, marrow and heart. Borax is itself toxic to infants in repeated doses and is no longer used for this in children. This must not be prepared or given.** For the condition and its modern treatment, see `bomyeong-san`. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
