Donguibogam 東醫寶鑑

治肉中腫起生瘤漸大. 白斂 一兩, 大黃ㆍ川芎ㆍ赤芍藥ㆍ黃芩ㆍ黃連ㆍ當歸ㆍ白礬 各五錢, 吳茱萸 二錢半. 右爲末, 雞子黃調勻, 攤帛貼之. 《類聚》

Translation

Treats a swelling risen in the flesh, a tumour growing gradually. Ampelopsis, 1 nyang; rhubarb, chuanxiong, red peony, scutellaria, coptis, Korean angelica and alum, 5 don each; evodia, 2 don 5 fun. Powder the above, work it even with egg yolk, spread it on silk and stick it on. — Yuchwi 類聚 (Donguibogam · Jechang)

Clinical commentary 臨床

A cooling plaster, and the mildest of the four 瘤 remedies. Ampelopsis at double the others heads a list that is almost entirely the 淸熱 cabinet — rhubarb, scutellaria, coptis — with red peony, chuanxiong and Korean angelica to move the blood under the lump and alum to draw and bind. Evodia at a quarter weight is the single warm drug and is there to carry the rest into the tissue. Egg yolk makes the paste and keeps it supple on silk. 肉中腫起生瘤漸大 is the indication, and the emphasis is on 漸大, growing gradually. The 類聚 is treating a lump that is still getting bigger, which in the chapter's scheme is the moment before it becomes one of the six 瘤 that must not be opened. Nothing spread on unbroken skin dissolves a lipoma, a fibroma or a cyst; the drugs do not cross the epidermis in any quantity and the lump is not inflammatory. What a plaster of this kind can do is reduce the redness and tenderness of an inflamed lesion, which may be why it was thought to help. **An enlarging soft-tissue mass is the one presentation that should not be watched under a dressing.** A lump that grows, is deeper than the fascia, is larger than a few centimetres or is painless and firm needs imaging and a biopsy, because that is how a soft-tissue sarcoma presents. — Eui Young (Justin) Chung, L.Ac., Dipl.
