Quicken the Blood and Dispel Wind Powder

Hwalhyeolgupung-san

한글 활혈구풍산 (신장풍창)  ·  Pinyin Huo Xue Qu Feng San

活血驅風散

杜油 is not a drug name. The collation line 油 : 冲 sits at the foot of the following 雞心散 leaf rather than this one, and the character 油 does occur there in 淸油, but 淸冲 is not a reading; the word it annotates is this 杜油, for which the 初刊本 prints 杜冲, the common variant writing of 杜仲. Eucommia is therefore the drug registered, and the transcription form 杜油 is printed in the source text as the 底本 rule requires. The 直指 formula of the same name registered as hwalhyeolgupung-san is a near relation rather than this prescription: it carries 槐實, which this leaf lacks, writes 桂皮 for 肉桂, and gives the two weight groups as 6 pun and 3 pun where this leaf gives 6 pun and 5 pun.

Translation

Treats the kidney-visceral wind sore with itching and pain; this comes of liver and kidney vacuity being invaded by wind-damp. Atractylodes lancea (stir-fried), eucommia (stir-fried in ginger juice), cinnamon bark, gastrodia, coix, red tangerine peel, areca, magnolia bark and bitter orange, 6 pun each; Korean angelica, cnidium, dahurian angelica, asarum, tribulus (stir-fried), peach kernel, white peony, pinellia, flying-squirrel droppings and licorice, 5 pun each. Cut the above as one packet, decoct it together with 5 slices of ginger and 2 jujubes, stir in a little powdered frankincense, and take on an empty stomach. — Deukhyo 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

Nineteen drugs in very small weights, the only formula in this group taken internally, and the one the chapter names first at 2.12 — 腎藏風宜服活血驅風散, 外用雞心散. The reasoning is given in the leaf itself: 此由肝腎虛爲風濕所侵也, a vacuity of liver and kidney that lets wind-damp in. So the blood medicinals of Samul-tang run through a set of wind drugs, with eucommia and cinnamon warming the lower body and the aromatics moving the middle, and the frankincense stirred in at the end for pain. 腎臟風瘡 at 2.12 opens with 兩足時熱, 脚跟作痛 and scaling sores of the inner shin that spread upward — burning feet and aching heels alongside a slowly advancing eruption of the lower legs. Taken together in a patient the book reads as 腎虛, that is a peripheral neuropathy and a stasis dermatitis side by side, and the modern reading is diabetes. A decoction of this kind would have relieved nothing of the underlying disease, though the formula is at least aimed at the whole patient rather than at the skin. **Contains pinellia, which is toxic in the raw state and must be properly processed, and asarum, which carries aristolochic acid in the aerial parts and should not be used in quantity or over long periods.** The weights here are small, 2 to 2.4 g a drug, which keeps the exposure low. — Eui Young (Justin) Chung, L.Ac., Dipl.
