Great Purslane Salve

Daemachi-go

한글 대마치고  ·  Pinyin Da Ma Chi Gao

大馬齒膏

The collation line 前隂 : 風門 stands at the foot of this leaf, at the page break before [051a], but the word it annotates is not in this leaf: it is the 方見前陰 cross-reference to 四生散 in the 腎臟風瘡 section text some seven lines earlier, where the 初刊本 reads 方見風門 instead. It is recorded here only because it falls inside this leaf, and no change has been made on its authority. 桐油, tung oil, has no identifier in the herb list; the leaf gives sesame oil as the alternative and that is what is registered.

Translation

Treats the blood wind sore of both legs. Purslane dried over gentle heat, 5 don; red lead, phellodendron, calcined alum and catechu, 3 don each; calomel 1 don. Powder the above, stir it evenly with tung oil or sesame oil, spread it on oiled paper; first irrigate the sore with the medicinal wash, and after blotting it dry, lay the paper on. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

Purslane dried and powdered leads, with phellodendron and catechu for the tannins and calcined alum for the drying, carried in oil and spread on paper rather than applied direct. The sequence matters more than the drugs: irrigate, blot dry, then dress — which is the order a wound clinic still uses, and the oiled paper keeps the powder from bonding to the surface. 血風瘡 is described at 2.12 as of a kind with 腎臟風瘡, from wind-heat and dryness of the blood in the three yin channels, with 瘙痒不常 and 膿水淋漓 and night sweats. The picture — both lower legs, itching in bouts, weeping, years long — is chronic eczema of the shins, usually with venous stasis behind it, and the formula is treating the surface of a problem that is in the veins. **Contains red lead and calomel, lead and mercury salts, and must not be prepared.** On eczematous skin over both legs the absorbing area is very large, and calomel applied to dermatitis is a documented cause of mercury poisoning. Purslane, phellodendron and catechu without the metals are a reasonable astringent dressing. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
