Translation

Treats sores arising on the foot, ulcerated and foul. Scorpion 7; blister beetle 14; croton kernels 14; areca 1 nut; sesame oil 1 nyang 5 don. Fry the above over a slow fire, putting in the scorpions first, then the beetles, then the croton, then the areca; when the croton is seen to have blackened, put in 1 nyang of wax, and once it has melted strain off the dregs and take only the waxed oil. Into it put roasted phellodendron and cnidium fruit, 2 don each; realgar, sulphur, red lead and cuttlefish bone, 1 don each; sweetgum resin, coptis, apricot kernel and calomel, half a don each — powdered, and stirred evenly into the waxed oil. Wash the sore with the medicinal wash and apply it; it takes effect at once. — Deukhyo 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

Four animal and purging drugs fried in sesame oil until spent, the dregs strained out and only the medicated oil kept, stiffened with wax and then loaded with twelve powders. The frying order is given exactly and is the whole method: the scorpion, beetle and croton give their principles up into the oil and the solids are discarded, so what reaches the skin is an extract rather than the drugs themselves. The chapter prescribes it at 2.12 for 下疰瘡, and the description there is precise — 其瘡口狹, 皮內極闊, 皮薄如竹膜, a narrow opening over a wide cavity under skin as thin as bamboo membrane. That is an undermined ulcer, and no ointment applied through the opening can reach the cavity; it has to be laid open first. 積年不愈 is the consequence of treating it from outside, and this formula could not have cured one. **Contains blister beetle (cantharidin, a vesicant and renal toxin), croton, scorpion, realgar (arsenic sulphide), red lead and calomel. This must not be prepared.** Five separate severe toxins, three of them metals or metalloids, applied to an open and infected foot ulcer: of everything in this group of leaves this is the one with the highest potential to kill the patient it was given to. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
