Donguibogam 東醫寶鑑

治歷節風, 百節痠痛無定處, 久則變成風毒, 痛入骨髓ㆍ不移其處. 又云骨髓痛者宜此藥. 虎骨(酥灸) 二兩, 白花蛇肉ㆍ天麻ㆍ防風ㆍ牛膝ㆍ白殭蚕(炒)ㆍ當歸(酒浸)ㆍ乳香ㆍ桂心各一兩, 全蝎(灸)ㆍ甘草(灸) 各五錢, 麝香一錢. 右爲末, 每二錢, 溫酒調下, 豆淋酒尤好. 《濟生》 ○一方加有自然銅ㆍ白附子ㆍ檳榔ㆍ羌活ㆍ白芷ㆍ川芎各一兩, 地龍ㆍ沒藥ㆍ雄黃各五錢. 服法亦同, 治白虎歷節風走痛. 《直指》

Translation

Treats joint-running wind with aching in all the hundred joints and no fixed place, which in time turns into wind toxin and the pain enters the bone marrow and does not shift from its place. It is also said that for pain in the bone marrow this medicine is appropriate. Tiger bone roasted with clarified butter, 2 nyang; pit viper flesh, gastrodia, saposhnikovia, achyranthes, stir-fried silkworm, wine-steeped tangkuei, frankincense and cassia heart, 1 nyang each; roasted whole scorpion and honey-roasted licorice, 5 don each; musk, 1 don. Powder; 2 don each time, taken stirred into warm wine, and black soybean wine is better still. One version adds pyrite, typhonium, areca, notopterygium, angelica dahurica and chuanxiong, 1 nyang each, and earthworm, myrrh and realgar, 5 don each; the method of taking is the same, and it treats white tiger joint-running wind with wandering pain. — Jesaengbang 濟生 · Injae Jijibang 直指 (Donguibogam · Pung)

Clinical commentary 臨床

**This must not be prepared.** Tiger bone is prohibited under CITES Appendix I, pit viper and musk are listed, and the variant adds realgar. The clinical observation in the indication is worth separating from all that: joint pain that moves at first and in time settles into a fixed place and enters the bone. That is the natural history of an inflammatory arthritis progressing to erosive disease, and the tradition marked the transition by calling the later stage wind toxin.
