The leaf writes 檳榔五箇爲末 before the other two drugs and 右爲末 again after them; the first 爲末 is set in parentheses as a preparation on the areca, which is how the corpus handles a preparation that precedes the collective instruction. 外腎, the outer kidney, is the scrotum and genitals.

Translation

Treats sores breaking out all over the body when a hundred drugs have had no effect. Five areca nuts (powdered), sulphur (ground raw) 5 don, calomel 5 fun. Powder the above; for each dose put 1 don in the palm of the hand, work it up with oil, and at bedtime rub it on the genitals. Do not wash the hands — only rub them until they are dry — and in a day or two it is cured. — Seui Deukhyobang 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

Three drugs, two of them metallic, and a route of application that tells you what the disease was. Sulphur at 5 don is the bulk of it, areca is the worm drug, calomel the mercury salt. The dose is a don worked up with oil in the palm and rubbed onto the genitals at night — and the hands are deliberately left unwashed and rubbed dry, so that the drug stays on the fingers as well. That is a scabies treatment. 遍身生瘡 that a hundred drugs have not touched, worst at night, treated by rubbing sulphur into the genital skin and the hands: the two sites scabies spares least are the genitals and the finger webs, and sulphur ointment is still a recognised scabicide, used at 5 to 10 per cent in petrolatum where permethrin is unavailable. Of everything in this section, this is the formula most likely to have worked. **Contains calomel, a mercury salt, rubbed into genital skin and left on the hands overnight. Mercury is absorbed through skin and far more readily through the thin skin of the genitals, and the chapter elsewhere records mercury poisoning as a complication of its own treatments. This must not be prepared.** The sulphur is the active part and needs no mercury beside it. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
