Donguibogam 東醫寶鑑

通治喘嗽. 葶藶子一兩(隔紙熬令黑)ㆍ知母ㆍ貝母各一兩. 右同爲末, 以棗肉半兩ㆍ別消砂糖一兩半, 和勻爲丸如彈子大, 新綿裹一丸含之, 徐徐嚥津, 甚者不過三丸效. 《本草》

Translation

Treats panting and cough generally. Lepidium seed, 1 nyang, parched black over a sheet of paper; anemarrhena and fritillaria, 1 nyang each. Grind them together to a powder and make pills the size of a bullet with half a nyang of jujube flesh and 1 nyang 5 fun of sugar melted separately; wrap one pill in fresh cotton wool, hold it in the mouth and swallow the saliva slowly. In a severe case not more than three pills are needed to work. — Jeungnyu Boncho 本草 (Donguibogam · Haesu)

Clinical commentary 臨床

A lozenge wrapped in cotton wool and held in the mouth while the sweetened juice is swallowed by degrees. The design is the whole point: three bitter drugs for a hot phlegmy cough, carried in sugar and jujube so that they can be kept in the mouth, and delivered to the throat continuously rather than in a single swallow. It is the same device as Soranajo-hwan's cherry-sized pill, and both are closer to a modern pastille than to a decoction. 隔紙熬令黑, parched black over a sheet of paper, is a processing instruction meant to blunt lepidium's harshness — the drug is a strong diuretic and purgative in its raw state. — Eui Young (Justin) Chung, L.Ac., Dipl.
