Paste method, and the chapter's most laborious preparation: phellodendron and anemarrhena are first decocted with three bowls of boys' urine, three of water, and a handful of arborvitae leaf, reduced to three or four bowls and strained; the two asparagi and both rehmannias go into that liquid with two more bowls of water, decocted and strained, then pounded to a mud, decocted again with a bowl or two of water and wrung for their juice, which joins the first. Apricot and walnut kernels are ground with water and filtered, ground and filtered again until no dregs remain, then added with the honey to the combined liquid and simmered down to a paste. The finished paste is set in water to draw off its fire toxin. Three to five spoonfuls per dose, stirred into an arborvitae-leaf decoction, on an empty stomach. Copper and iron vessels are to be avoided. Note on modern practice: boys' urine (童便) is shown as recorded, for textual fidelity.

Donguibogam 東醫寶鑑

治陰虛火動, 潮熱盜汗, 咯唾血, 欲成勞瘵疾. 黃柏ㆍ知母 各四兩, 生地黃ㆍ熟地黃ㆍ天門冬ㆍ麥門冬 各二兩, 杏仁 七錢, 胡桃仁ㆍ蜂蜜 各四兩. 右剉, 先將黃柏ㆍ知母, 以童尿三椀ㆍ水三椀ㆍ側柏葉一把, 煎至三四椀去滓. 又將天ㆍ麥門冬ㆍ生熟地黃, 入汁內, 添水二椀, 煎汁去滓, 再擣爛如泥, 另用水一二椀, 煎熬絞取汁, 入前汁. 將杏仁ㆍ胡桃仁, 用水擂爛濾汁, 再擂再濾至無滓, 同蜜入煎汁內, 火熬成膏, 入水內去火毒. 每服三五匙, 以側柏葉湯調, 空心服. 忌銅鐵器. 《醫鑑》

Translation

Treats Yin vacuity with fire stirring: tidal heat, night sweats, blood expectorated and spat, consumption about to form. Phellodendron and anemarrhena, 4 nyang each; raw rehmannia, prepared rehmannia, asparagus root, and ophiopogon, 2 nyang each; apricot kernel 7 don; walnut kernel and honey, 4 nyang each. Cut the above. First decoct the phellodendron and anemarrhena with three bowls of boys' urine, three bowls of water, and a handful of arborvitae leaf, reducing to three or four bowls, and strain. Then add the two asparagi and both rehmannias to that liquid with two more bowls of water, decoct, strain, pound the dregs to a mud, decoct again with a bowl or two of water and wring out the juice, adding it to the former. Grind the apricot and walnut kernels with water and filter, grinding and filtering again until no dregs remain; add these with the honey to the combined liquid and simmer over fire into a paste. Set the paste in water to remove its fire toxin. Take three to five spoonfuls each time, stirred into an arborvitae-leaf decoction, on an empty stomach. Clinical commentary 臨床

The vacuous-fire cabinet's magnum opus — a paste built in four separate extractions and set in water afterward to let its own fire escape. Walnut and apricot kernels give the oil that a dry body cannot make for itself; the honey makes a year's medicine keep.
