Clinical commentary 臨床

The first half of a matched pair with Geopung-jeseup-tang, and the pairing is the chapter's most quoted piece of reasoning: the left side belongs to blood, the right side to qi. Left-sided paralysis is therefore blood vacuity with dead blood, and the formula is the four-substance decoction enlarged with peach kernel and safflower to move the stasis, phlegm drugs for the wind, and achyranthes to lead it all down into the legs. The lateralization is anatomically wrong — hemiplegia follows the lesion, not the blood — but the clinical instinct underneath it, that a wasted flaccid limb needs nourishing and a heavy congested one needs moving, is sound. Twenty-one drugs with twenty processing instructions is also a fair portrait of late Ming pharmacy. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
