Donguibogam 東醫寶鑑

治老人痰結血枯, 成膈噎. 當歸ㆍ白芍藥(炒)ㆍ熟地黃ㆍ白茯苓各一錢, 黃連(以吳茱萸同炒, 去茱萸) 八分, 貝母(炒)ㆍ瓜蔞仁ㆍ枳實ㆍ陳皮ㆍ厚朴ㆍ香附子ㆍ川芎ㆍ蘇子各七分, 沉香(水磨取汁) 五分. 右剉, 作一貼, 薑一ㆍ棗二, 水煎, 入沉香汁ㆍ竹瀝調服. 《回春》

Translation

Treats diaphragm block and gullet block in the old, formed from phlegm binding and withered blood. Chinese angelica, fried white peony, prepared rehmannia and white poria, 1 don each; coptis fried together with evodia, the evodia then discarded, 8 fun; fried fritillaria, trichosanthes seed, bitter orange, tangerine peel, magnolia bark, cyperus, chuanxiong and perilla seed, 7 fun each; aquilaria ground in water for its juice, 5 fun. Cut up, make one packet, add one slice of ginger and two jujubes, decoct in water, and take with the aquilaria juice and bamboo sap stirred in. — Manbyeong Hoechun 回春 (Donguibogam · Guto)

Clinical commentary 臨床

Samul-tang with prepared rehmannia at its centre, and phlegm-moving drugs built around it: the diagnosis is 痰結血枯, phlegm bound on withered blood, and the treatment answers both halves at once. Aquilaria is not decocted but ground in water and the juice added at the end, which is how the Bogam handles aromatic resins whose volatile part would be lost to boiling. Coptis fried with evodia and the evodia then thrown away is a technique worth noticing: what is wanted is the cold drug carrying a trace of the hot one, so that it does not chill a stomach that is already refusing food.
