Major Sophora Root Pill

Daegosam-hwan

한글 대고삼환  ·  Pinyin Da Ku Shen Wan

大苦參丸

A collation note stands in the transcription a few lines below this leaf, at the page boundary before [046b], reading 文 : 末(初刊本). The word it annotates is not in this leaf: it is the 文 of 卽取貝母文和水灌入口中 in the 人面瘡 head text above, which the 初刊本 reads 末 — powdered fritillary, which is what the sentence requires. The note is recorded here because this is the only formula leaf of the 人面瘡 section; it belongs to that head passage, not to this entry, and nothing is changed in the text above.

Clinical commentary 臨床

Nineteen drugs, built on 2 nyang of sophora root against 5 don of everything else, and the body of it is a wind-dispersing list: saposhnikovia, schizonepeta, angelica root, notopterygium, pubescent angelica, vitex fruit and tribulus, with the three aconites — raw 川烏, 白附子 and blast-fried 草烏 — behind them for penetration, and blood drugs and astragalus to support. The dose is large, fifty to seventy pills, and the leaf makes a courteous allowance: wine as the vehicle, or tea for the patient who does not drink. 人面瘡 is one of the strangest heads in the book. The section says such a sore grows on the knee or the arm with eyes, mouth and nose all formed, that old books call it the fruit of karmic wrong and the patient must purify the mind and repent, and it records the case in which a sore on a man's upper arm ate whatever was fed to it and flushed red when given wine, until fritillary powder was poured in and it drew its brows together, shut its mouth and crusted over. What is being described is most plausibly a large chronic granulating ulcer whose irregular surface was read as a face. The pairing of the indication with 膁瘡, the shin ulcer, points the same way: this is the medicine for a chronic ulcer of a limb that will not close. **Contains 川烏 raw, 草烏 and 白附子 — three aconite drugs, which are cardiotoxic, in a pill taken fifty to seventy at a time. This must not be prepared or taken.** Raw 川烏 is the dangerous form, and aconite poisoning begins with perioral numbness and progresses to ventricular arrhythmia. A chronic leg ulcer needs assessment of its arterial and venous supply, of diabetes, and of malignancy. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
