Black Soybean and Licorice Decoction

Daedugamcho-tang

한글 대두감초탕  ·  Pinyin Da Dou Gan Cao Tang

大豆甘草湯

The transcription writes 赤皮䓗; 䓗 is normalized to 葱. 槐條, pagoda-tree twigs, has no entry in the herb list — 槐皮, 槐花 and 槐角子 are registered but not the twig — so it is left out of the ingredient list and named here. 一合 and 一握 are volume and handful measures and take no gram figure.

Donguibogam 東醫寶鑑

治陰蝕下疳瘡. 甘草 一兩, 赤皮葱 三莖, 黑豆 一合, 槐條 一握. 右濃煎取淸, 候溫浸浴, 日二. 《精義》

Translation

Treats the eating sore of the genitals and chancre. Licorice 1 nyang, red-skinned scallion three stalks, black soybean 1 hop, pagoda-tree twigs one handful. Decoct the above thick, take off the clear liquor, and when it has cooled to a comfortable warmth soak the parts in it, twice a day. — Jeongui 精義 (Donguibogam · Jechang)

Clinical commentary 臨床

A sitz bath of four cheap kitchen things: licorice, red-skinned scallion, black soybean and a handful of pagoda-tree twigs, boiled down hard, decanted clear, and used at body temperature. Licorice at a full nyang is the bulk of it and is the one ingredient with a defensible local action — glycyrrhizin is mildly anti-inflammatory and licorice decoctions are soothing on raw mucosa. The black soybean is the chapter's standing 解毒 drug and the scallion and twigs supply the warm aromatics. The leaf is named in the 陰蝕瘡 treatment line as the first of three steps — 以大豆甘草湯漬之, 洗毒散煎湯浴之, 磨風膏溫潤之 — soak, then bathe, then anoint. Soaking an ulcerated glans in a bland warm decoction twice a day is reasonable supportive care and would have given real relief, especially where the foreskin is swollen and cannot be retracted. 候溫, waiting until it is only warm, is the instruction that keeps it safe. As with every wash in this section, the bath treats the surface and not the infection. **An ulcerating genital lesion needs testing and systemic treatment; soaking is comfort care. Do not use a hot decoction on an ulcer, and do not share the basin.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
