Chancre Wash Decoction

Segam-tang

한글 세감탕  ·  Pinyin Xi Gan Tang

洗疳湯

The transcription writes 川鍊子; the 初刊本 reads 練, and the collation note 鍊 : 練 stands on its own line at the 047a/047b page boundary two leaves below. The transcription's reading is printed here under the project's 底本 rule, with the variant recorded; the corpus prints 川練子 for the same drug elsewhere and the drug is entered under cheollyeonja. 醫鍳 is normalized to 醫鑑. 葱根, the roots of the scallion, is entered under 生葱. The 靑布, the blue cloth the liquor is carried on, is the applicator and is kept in the method rather than as an ingredient.

Clinical commentary 臨床

Six drugs in equal parts, boiled, and the hot liquor swabbed over the sore with a cloth. Toosendan fruit and coptis are the bitter, cooling pair that carry the chapter's reading of 下疳瘡 as damp heat sunk into the lower burner; rock pine, Sichuan pepper, scallion root and mugwort leaf are the warm, aromatic, mildly antiseptic half that was expected to dry the surface. This is the soundest local treatment the chapter has for a genital ulcer, and the chapter says so itself in the head of the wash section: 常須藥湯淋洗, 以去膿汁惡水 — one must always irrigate with a medicinal decoction to take away the pus and the foul water. Keeping an ulcerated glans clean and then dry is still essentially the whole of local care; everything a modern clinician adds is systemic. Whether the six drugs do anything beyond what warm water and a clean cloth would do is doubtful, and it does not much matter, because the irrigation is the active part. What the wash cannot do is treat the disease. A chancre is a sign of something transmitted, and the chapter knows it — it warns a few lines above that a chancre that will not heal 必成楊梅瘡, becomes syphilis. **A genital ulcer needs serological and swab testing and systemic antibiotic treatment; washing it is supportive care and nothing more, and the partner needs treating too.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
