Chase Out Wind Powder for Great Wind Sores

Chupung-san

한글 추풍산 (제창)  ·  Pinyin Zhui Feng San

追風散

The Donguibogam has two formulas named 追風散. The other, from the Jikji, is the nineteen-drug head-wind powder built on 川烏ㆍ石膏ㆍ白殭蠶ㆍ川芎ㆍ防風ㆍ荊芥ㆍ甘草 registered as chupung-san; this three-drug purge in the sores chapter shares nothing with it but the name and so takes the chapter tail. The 大風瘡 body text at 019b names this formula 追虱散, and the transcription carries the collation note at the page foot that the 初刊本 reads 風 where the transcription reads 虱, so the first edition has 追風散 there; the leaf head is 追風散 and that is the name registered. The transcription writes 欝金 for 鬱金 and 虫 for 蟲; the corpus forms are printed. 蟬肚者, the cicada-belly grade of curcuma, is base text and not an interlinear note, so it is given in parentheses as a substance gloss. 大風油 is the pressed oil of 大風子 and has no herb entry of its own; it is recorded under daepungja, following the corpus treatment of 胡麻油 under homa.

Translation

Taken on the second day; it drains the blood and drives out the worms. Brocade-veined rhubarb 6 nyang; curcuma tuber of the cicada-belly grade 1 nyang 8 don; gleditsia spine, stir-fried, 1 nyang 5 don. Powder the above. For the first dose take 5 don or 6 don, add chaulmoogra oil 1 don 5 fun and a little crude mirabilite, dissolve the whole in a bowl of good warm wine, and take it at the fifth watch on an empty stomach. Wait until the hour of the dragon and stir up another bowl as before, adding a little boiled honey, without letting the patient know. First rinse the mouth clean with water, then take the medicine, and be sure to clear the mouth afterwards with honey. On no account let the patient lie down. After a good while there will be a painful purging several times over, which is not to be feared; restore the patient with a thin gruel. ○ This medicine is hard to treat the old and weak with; those under fifty may be treated. The vigorous and strong take three doses in ten days — that is, on the first of the first month take Sopung-san, on the second Chupung-san, on the third Mapung-hwan, and so round again from the beginning. The thin and weak take one dose in ten days. — Yuchwi 類聚 (Donguibogam · Jechang)

Clinical commentary 臨床

Day two, and the whole formula is a purge: rhubarb at six nyang, curcuma, and stir-fried gleditsia spine, with chaulmoogra oil and a pinch of mirabilite stirred in at the time of dosing. 瀉血追蟲 — drain the blood, chase out the worms — is the stated object, and the worms are meant literally; 通天再造散 in the next leaf has the practitioner inspect what comes out and read the colour for how old the disease is. The procedural detail is unusual and worth reading. The dose goes down at the fifth watch, before dawn; a second bowl follows at the hour of the dragon with honey stirred in 勿令患人知, without letting the patient know, because the first bowl will have made the second hard to face. The mouth is rinsed before and cleared with honey after, and the patient is on no account to lie down. 良久痛瀉數次不妨 concedes what is coming. **Contains rhubarb and crude mirabilite in purgative quantity, and chaulmoogra oil, which is toxic. This must not be prepared.** Chaulmoogra is the one drug in this chapter with a real history against leprosy — it was the standard treatment worldwide until the sulfones of the 1940s — but it is an irritant and hepatotoxic, and the dose here is incidental to a cathartic. A course of three such purges in ten days in a wasted patient is dangerous on its own account, which is why 老弱者難治 is in the leaf. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
