Donguibogam 東醫寶鑑

治咳上氣不得卧. 人參ㆍ陳皮ㆍ紫蘇葉ㆍ五味子. 右剉一兩, 薑三, 水煎服. 《丹心》

Translation

Treats cough with qi rising, so that the patient cannot lie down. Ginseng, tangerine peel, perilla leaf and schisandra. Cut up and take 1 nyang, with three slices of ginger, decocted in water. — Dansim 丹心 (Donguibogam · Haesu)

Clinical commentary 臨床

Four drugs and no weights, which is how the Dansim writes a formula meant to be easy to remember. 不得卧 — cannot lie down — is orthopnoea, and it is the sign that separates this from an ordinary cough: a patient who must sit up to breathe has fluid or failure behind the cough, not irritation. **Orthopnoea with cough needs assessment for heart failure and for lung disease.** The formula itself is mild — ginseng and schisandra to hold the qi, perilla leaf and tangerine peel to move it — and would not touch either cause. — Eui Young (Justin) Chung, L.Ac., Dipl.
