Donguibogam 東醫寶鑑

治傷食, 少食, 濕腫, 四肢無力, 酒色過度, 勞逸有傷, 骨熱痰火等證. 初服, 或作熱, 或瀉痰, 或作飽, 或善飢, 久服則輕身健骨. 蒼朮(泔浸去黑皮, 切片焙) 一斤, 入(蒸過)白茯苓 四兩(剉). 入水十椀熬, 取汁二椀, 濾去渣, 又入水熬取汁二椀, 將渣擣爛, 又入水熬絞取汁一二椀, 去渣, 都合前汁, 再熬至二椀, 加蜜 四兩熬至稠, 滴水成珠爲度, 日服二三次, 白湯送下. 此藥氣極雄壯, 通行脾腎二經. 《入門》

Translation

Treats food damage, poor appetite, damp swelling, the four limbs without strength, damage from drink and sex or from excess of labour and idleness, bone steaming with phlegm fire and the like. On first taking it there may be heat, or phlegm purged, or a sense of fullness, or ready hunger; taken long, the body is lightened and the bones strengthened. Atractylodes lancea 1 geun, steeped in rice-washing water with the black skin removed, sliced and dry-roasted; add 4 nyang of white poria, steamed and cut up. Put in ten bowls of water and reduce to two bowls of liquor, straining off the residue; add water and reduce again to two bowls; pound the residue to a pulp, add water and reduce once more, pressing out one or two bowls of liquor; discard the residue, combine all the liquors and reduce again to two bowls; add 4 nyang of honey and reduce until thick, until a drop forms a bead in water. Take two or three times a day, sent down with plain hot water. This medicine's qi is exceedingly powerful, and it travels through both the spleen and kidney channels. — Uihak Immun 入門 (Donguibogam · Naesang)

Clinical commentary 臨床

滴水成珠 — until a drop forms a bead in water — is the test for when the reduction is done, and it is precise: a syrup thick enough to hold its shape rather than disperse. The same test appears in the Accumulations chapter's paste preparations. The warning about first doses is unusual and worth keeping: heat, purged phlegm, fullness or sharp hunger may follow at the start. The source treats these as expected rather than as adverse, which is how it reads a medicine whose qi is 極雄壯, exceedingly powerful.
