A different formula from the 六磨湯 of the Stool chapter, which is the Deukhyo's 四磨湯 with rhubarb and bitter orange peel added for a heat-bound stool. This adds costus and bitter orange peel to the ginseng version instead. The transcription writes 氣膈 with 膈.

Translation

Treats the same as above. It also treats qi blockage in the chest when it is severe; to Four Ground Ingredients Decoction add costus and bitter orange peel in equal parts. The method of preparation and of taking it are as above. — Dongwon 東垣 (Donguibogam · Haesu)

Clinical commentary 臨床

Sama-tang with two more movers added when the chest itself feels blocked. Costus and bitter orange peel both work across the middle burner, so the enlarged formula reaches the fullness under the breastbone that the four-drug version leaves alone. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
