Four Milled Ingredients Decoction

Sama-tang

한글 사마탕  ·  Pinyin Si Mo Tang

四磨湯

磨 means the drug is ground against a wet stone so that the suspension rather than a decoction is taken; the brief boiling only warms it.

Donguibogam 東醫寶鑑

治氣滯大便秘澁. 大檳榔ㆍ沉香ㆍ木香ㆍ烏藥. 右四味等分, 各濃磨水, 取七分盞, 煎三五沸, 微溫服, 空心. 《得效》

Translation

Treats qi stagnation with the stool bound and inhibited. Large areca, aquilaria, costus root and lindera, the four in equal part. Mill each thick in water, take seven tenths of a cup, bring it to the boil three to five times, and take it barely warm on an empty stomach. — Seui Deukhyobang 得效 (Donguibogam · Daebyeon)

Clinical commentary 臨床

The milling method is the whole point. Four hard aromatic drugs are ground against a wet stone and the milky suspension drunk after the briefest boiling, so that the volatile oils — which a long decoction would lose — reach the patient intact. All four bear qi downward, and for a constipation that is counterflow rather than blockage this is the tradition's standard answer. The next entry adds two drugs for when there is heat as well. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
