Sage-Penetrating Five Treasures Elixir

Tongseongobo-dan

한글 통성오보단  ·  Pinyin Tong Sheng Wu Bao Dan

通聖五寶丹

The transcription prints a space inside 土茯苓 一斤 at 026b; it is not a gap in the text. 飛白霜 is left out of the ingredient list because it is not a registered drug and because the leaf itself only conjectures its identity: the closing clause 飛白霜疑是輕粉, "flying white frost is probably calomel", is the book's own editorial note and is kept as part of the leaf. The chapter index at 025a names this formula 通仙五寶丹, where the leaf heads itself 通聖五寶丹; the leaf's form is printed and the index reading recorded here.

Translation

Treats plum-blossom, heavenly blister and cotton-flower sores ulcerated open down to the bone; or aching of the sinews and bones; or lumps over the whole body; or red and white vitiligo and goose-foot tinea; or broken skin and rotten flesh with a stench of the mouth past bearing. Every stubborn sore and malign toxin is treated by it. Stalactite powder 3 fun, cinnabar 2 fun, amber and borneol 5 ri each, pearl 2 ri 5 ho. Grind fine; for each dose take 5 ri, and add separately 2 fun 5 ri of flying white frost (stir-fried) to make up one packet. Each day take a geun of smilax, decoct it in water and draw off ten bowls of the liquid; put one packet of the medicine into one bowl of the decoction, stir it evenly and take it in the morning. The whole of that smilax decoction must be drunk up within the day, and no other soup or water may be used. Take one course and it is cured — a marvel. It is the first immortal formula under heaven, in all ages, for plum-blossom sores. — Hoechun 回春 ○ Flying white frost is probably calomel. (Donguibogam · Jechang)

Clinical commentary 臨床

Five mineral and resinous treasures ground to a powder so fine that a dose is five ri — about two hundred milligrams — and even that is not the active part. The active part is the 飛白霜 added separately at each dose, two and a half fun of it, which the book's own note says is probably calomel. So the formula is a mercury powder dressed in stalactite, cinnabar, amber, borneol and pearl, and taken every morning in a bowl of smilax decoction, with a whole geun of smilax to be drunk through the day and nothing else to drink. The indication list is the fullest in the section and reads as a description of advanced syphilis: ulceration to the bone, bone pain, nodules over the body, depigmented patches, palmar lesions, and the foul breath of a mouth with gummatous destruction. The claim to be the first formula under heaven in all ages is the Hoechun's, not the compiler's. **Contains mercury. This must not be prepared or taken.** Calomel and the other mercury salts in this section are absorbed through the gut and through broken skin, and they accumulate: salivation, loosened teeth, a metallic taste, tremor, kidney injury and the nerve damage the chapter itself calls weakness and paralysis of the limbs. The chapter records this at 2.2 as a complication of its own treatment — mercury poisoning after taking quicksilver or calomel in error, with wind-heaps, swelling, ulceration and running pus. Several of the formulas in this section are offered as the cure for the poisoning the others cause. The cinnabar (丹砂) in the base is mercuric sulfide and adds to that load. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
