奪命散

不去油 — the oil not removed — is the opposite of the usual instruction for croton seed and makes this preparation far more drastic. Registered with the chapter suffix to distinguish it from the formulas of the same name elsewhere in the book.

Translation

Treats sudden wind stroke with drool rising, the qi blocked, the jaw clenched and the eyes staring; lockjaw with convulsion; and critical fright wind in children. Arisaema, sweet pepperweed seed, angelica dahurica, pinellia and croton seed with the husk removed but the oil left in, in equal parts. Powder; half a don each time, taken stirred into a mouthful of ginger juice, with immediate effect. Whenever the jaw is clenched and medicine will not go down, this should be used. — Uihak Jeonggam 醫鑑 (Donguibogam · Pung)

Clinical commentary 臨床

**This must not be prepared.** Croton seed with the oil deliberately left in is the most dangerous instruction in the chapter: croton oil is a vesicant and a violent purgative, lethal in small quantities, and the usual preparation presses it out. Here it is kept because the point is a drastic reflex response in a patient who cannot swallow. Half a don in a mouthful of ginger juice, stated as suitable for children, is a prescription that would kill a child. Recorded as history. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
