Snatch Back Life Powder (Injuries chapter)

Talmyeong-san

한글 탈명산  ·  Pinyin Duo Ming San

奪命散

Four other formulas called 奪命散 are already on this site, from the Buin, Inhu, Pung and Han chapters; none shares an ingredient with this one, and this is a fifth distinct prescription, registered with the chapter suffix. Lime is a processing agent for the leech and is not in the ingredient list. A second place must not be linked here: Damum 1.x lists 小兒方奪命散 — the Children chapter's formula of the same name — and that chapter is not yet translated, so the name there stays plain text. Buin's 催生 formula list likewise means talmyeong-san-buin, not this one. Five formulas in the corpus now carry this name.

Translation

Treats wounds from blades, and falling from a height, and crushing injury from timber or stone, with stagnant blood collected, pain in the heart region and abdomen, and neither stool nor urine passing. Leech, 5 don, mixed with lime and stir-fried to scorching; rhubarb and the first-grind powder of black morning-glory seed, 2 nyang each. Powder them; take 2 don at a time stirred down in hot wine, and if some hours pass with no effect, use one dose again, taking the issue of foul blood as the measure. — Deukhyo 得效 (Donguibogam · Jesang)

Clinical commentary 臨床

Three drugs, all of them drastic, and the logic is single-minded: the injury has left blood sitting where it should not be, so drive it out below. Rhubarb and black morning-glory seed are the purgatives, two nyang each, and the leech is there to break the clot itself. The instruction to repeat the dose if nothing has happened in a few hours, and to keep going until foul blood appears, makes the endpoint explicit. The patient described — fallen from a height, crushed under timber or stone, in abdominal pain and passing neither stool nor urine — is one in whom this treatment is actively dangerous. That picture is a pelvic or abdominal injury with bleeding into the retroperitoneum, a urethral injury, or a paralytic ileus, and such a patient is often already hypovolaemic. Purging them removes more fluid they cannot spare, and the leech, whose saliva contains hirudin, is an anticoagulant given to someone who may be bleeding internally. The foul blood the prescriber is told to wait for would be a sign of harm, not of cure. **Contains leech as an internal drug and black morning-glory seed, a drastic purgative. This must not be prepared.** A patient who cannot pass urine after a crush injury needs imaging and a catheter, not a cathartic. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
