Twin Aconites Powder

Ssango-san

한글 쌍오산  ·  Pinyin Shuang Wu San

雙烏散

The leaf carries two 初刊本 collation pairs at its foot, 𦙄 : 胷 and 㝎 : 定, but neither 𦙄 nor 㝎 occurs in this leaf: they belong to the leaves above it, which write 𦙄腹脇痛 (當歸鬚散) and 乳香㝎痛散. Both are glyph variants the house correction already covers.

Translation

Treats all injuries and the hundred hurts, where long afterwards there is pain from time to time; it also suits a fresh injury that is painful. Sichuan aconite and wild aconite (lightly blast-fried) 3 don each; Chinese angelica, white peony, sappanwood, rhubarb, dried fresh rehmannia and red yeast rice (dry-fried) 5 don each; a little musk. Powder them, put them into an earthenware bottle, boil them with wine, let it go cold and take it. If numbness is felt, there is no harm. But wild aconite used raw would be too violent, and so it is lightly blast-fried. — Immun 入門 (Donguibogam · Jesang)

Clinical commentary 臨床

Both aconites at 3 don, lightly blast-fried, then a blood-moving body at 5 don each — 當歸, 白芍藥, 蘇木, 大黃, 生乾地黃, 紅麴 — and a little musk. The whole powder is simmered in wine in an earthen bottle and drunk cold. The indication is the one real thing here: 久後時常疼痛, the ache that comes back long after the injury has otherwise closed. The text is unusually frank about what the patient will feel, and about why. 如覺麻痺無害 — if numbness comes on, no harm; and 但草烏生用恐太猛, 所以略炮 — raw wild aconite would be too violent, which is why it is roasted first. Read without the frame, that is a description of early aconite poisoning being accepted as the sign the dose has landed. The numbness is a sodium-channel blockade, and the same blockade in cardiac muscle is what kills. **Contains aconite, which is toxic. This must not be prepared.** It also contains musk, from a CITES-protected animal, and rhubarb, which purges — given to someone bleeding internally after a blow, a purgative makes matters worse, not better. — Eui Young (Justin) Chung, L.Ac., Dipl.
