Donguibogam 東醫寶鑑

治疥瘡痛痒. 乾葛 二錢, 陳皮ㆍ甘草 各一錢半, 升麻ㆍ蒼朮ㆍ桔梗 各一錢, 赤芍藥 七分半, 大黃 五分, 半夏ㆍ當歸ㆍ茯苓ㆍ白芷 各三分, 乾薑ㆍ枳殼 各二分. 右剉, 作一貼, 入薑五片ㆍ燈心十五莖, 水煎服. 《入門》

Translation

Treats the pain and itch of scabies sores. Dried pueraria root 2 don; aged tangerine peel and licorice, 1 don 5 fun each; cimicifuga, atractylodes lancea and platycodon, 1 don each; red peony 7 fun 5 ri; rhubarb 5 fun; pinellia, Korean angelica, poria and dahurian angelica, 3 fun each; dried ginger and bitter orange, 2 fun each. Cut the above and make one packet; add five slices of fresh ginger and fifteen stalks of rush pith, decoct in water and drink. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The section treats scabies from the outside with five powders and from the inside with this, named first among the 治疥 formulas. Its core is 升麻葛根湯 — cimicifuga, pueraria root, red peony and licorice — with a second layer of tangerine peel, atractylodes, platycodon, poria, pinellia and bitter orange moving qi and damp, a little rhubarb to move the bowel, and angelica for the blood. 和氣, harmonizing the qi, is the claim the additions make on top of the base. The reasoning comes from the section's own doctrine: 疥癬皆血分熱燥以致風毒剋於皮膚, both scabies and ringworm arise from heat and dryness in the blood division letting wind toxin strike the skin, 浮淺者爲疥深沉者爲癬, the shallow kind being scabies and the deep-set kind ringworm. Treated that way, an itching eruption is a systemic matter and an internal decoction follows. **Contains pinellia, which is toxic raw and must be properly processed before use.** The honest reading is that scabies is an infestation by Sarcoptes scabiei, a mite living in the stratum corneum, and that nothing taken by mouth in this formula reaches it — which is not a judgement on the formula so much as on the premise. Ivermectin, taken by mouth, is the one internal treatment that does work, and it works by killing the mite. A decoction like this one may have eased the itch; the itch of scabies is a hypersensitivity response and does continue for weeks after the mite is dead, which is worth knowing before a patient is treated twice over for a cure that has already happened. — Eui Young (Justin) Chung, L.Ac., Dipl.
