Donguibogam 東醫寶鑑

治乾疥及春月發者, 以此開鬱. 蛇床子 三錢, 寒水石 二錢半, 白礬ㆍ吳茱萸 各二錢, 黃栢ㆍ大黃ㆍ硫黃ㆍ輕粉 各一錢, 樟腦 半錢, 檳榔 一箇. 右爲末, 香油調付. 《入門》

Translation

Treats dry scabies, and the kind that breaks out in the spring months, by opening the constraint. Cnidium fruit 3 don, glauberite 2 don 5 fun, alum and evodia 2 don each, phellodendron, rhubarb, sulphur and calomel 1 don each, camphor half a don, one areca nut. Powder the above and work it up with sesame oil to apply. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The first of the five 疥 formulas, matched to the first of the five kinds: 乾疥, dry scabies, 皮枯屑起 — the skin withered and the scurf rising. Cnidium fruit leads, with evodia, alum and glauberite, and the seasonal note is part of the indication: 春月發者, the outbreak that comes in spring. Scabies does run in seasonal waves, and spring presentations of a long-standing infestation are the dry, scaling, heavily scratched kind this names. 疥癬 is scabies and the ringworms, and the chapter is clear that the itch of 虫疥 spreads by contagion. Of the ten drugs here, one works. **Sulphur is a genuine scabicide — precipitated sulphur in a 5 to 10 per cent ointment is still used, and is the treatment of choice in infants and in pregnancy, where permethrin and ivermectin are avoided.** Cnidium fruit and camphor relieve itch, which is not the same thing as killing the mite. **Contains calomel, which is mercurous chloride, spread in oil over broken and excoriated skin, where mercury is absorbed; mercury applied to scabies caused chronic poisoning for as long as it was used. Camphor is also absorbed through skin and is neurotoxic in children. This must not be prepared.** Scabies is curable: topical permethrin, or oral ivermectin, with all close contacts treated at the same time and bedding and clothing laundered hot. — Eui Young (Justin) Chung, L.Ac., Dipl.
