Donguibogam 東醫寶鑑

治大便閉塞. 巴豆(去殼)ㆍ乾薑ㆍ韭子ㆍ良薑ㆍ硫黃ㆍ甘遂ㆍ白檳榔等分. 右爲末, 飯丸如雞子黃大, 早朝先以椒湯洗手, 麻油塗手掌, 握藥一粒, 移時便瀉. 欲止則以冷水洗手. 《本事》

Translation

Treats the stool shut and blocked. Shelled croton seed, dried ginger, leek seed, lesser galangal, sulphur, kansui and white areca, equal parts. Powder, make pills with cooked rice the size of an egg yolk; early in the morning first wash the hands in a zanthoxylum decoction, rub sesame oil on the palm, and hold one pill — in a little while the bowel moves. To stop it, wash the hands in cold water. — Bonsabang 本事 (Donguibogam · Daebyeon)

Clinical commentary 臨床

The pill is never swallowed. It is held in an oiled palm and the bowel is said to move; washing the hands stops it. Transdermal absorption of croton oil through oiled skin is real enough to cause severe irritation, but nothing here reaches the gut, and the purgation described cannot have happened by the route claimed. What the entry does record is a serious attempt at a controllable dose — an application that can be removed — which is the same instinct as the water-counting in the Numinous Treasure Elixir.
