Rouse the Spleen Powder

Seongbi-san

한글 성비산  ·  Pinyin Xing Pi San

醒脾散

The transcription writes 白殭蚕, which the corpus prints in that form, and 昏沉 with 沉 rather than 沈, likewise kept as printed. 二分半 is entered as 1 g, following the chapter's weight table. The section prose at the head of 1.30 prescribes this leaf twice, once with 加味朮附湯 and 釀乳方 for the half-yin half-yang case and once with 蟬蝎散 where yang signs remain.

Clinical commentary 臨床

The four gentlemen — ginseng, atractylodes, poria, licorice — with costus root to move the stomach, and then the wind quartet: typhonium, stiff silkworm, gastrodia and scorpion. Five fun of each is a very small weight, and the striking instruction is the last one: 漸漸服, 不可頓服, give it gradually and never all at once. That instruction is good. A child vomiting repeatedly will bring back a large drink and keep down a small one, and giving fluid in small frequent sips rather than in a single volume is exactly what oral rehydration teaching says today. The chapter arrived at the method by a different route — it is thinking about not overwhelming an exhausted spleen — but the practice is right. The rest of it is not. For what the drowsy, vomiting, twitching child has and what would have reached it, see the note on `baekchul-san-soa`. **Contains typhonium (白附子), a toxic tuber, and scorpion, a neurotoxic animal venom, given to a child; the text gives no dose by weight for the patient anywhere. The weights here are small, which is the one mitigating fact, but dried scorpion cannot be standardised for potency and an infant has roughly 250 ml of blood, an immature blood–brain barrier and poor renal clearance. This must not be prepared or given.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
