Ivory Pill

Sanga-hwan

한글 상아환  ·  Pinyin Xiang Ya Wan

象牙丸

象牙, ivory, is not a registered drug and is left out of the ingredient list; it is the drug the formula is named for, at 3 don. The transcription writes 猬皮 where the drug dictionary heads the entry 蝟皮, and prints 並 rather than 幷; the transcription's forms are followed, as the corpus does not normalize that pair.

Translation

Treats plum-blossom sores that have turned into a fistula. Ivory 3 don; one soft-shelled turtle shell and one hedgehog skin, both burnt. Grind to powder, bind with date flesh into pills the size of a cherry, and take one pill on an empty stomach, dissolved down in boy's urine; clear tea will also do. After seven days, take the powder of the three drugs, mix it with pig bile and apply it on the sore. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

Three hard animal materials, two of them burnt to a char, bound with date flesh — the book's standing approach to a tract that will not close. Turtle shell and hedgehog skin are both drugs it uses for sinking masses and for haemorrhoidal and fistulous disease, and burning them was held to turn them toward closing a sore rather than softening it. The same powder is then used both ways: swallowed for seven days, then mixed with pig bile and laid on the opening. A syphilitic fistula is a tract through destroyed tissue, often through bone, and nothing here would close it. Boy's urine as the vehicle is the chapter's usual choice for a toxin thought to need cooling and leading downward. **Ivory (象牙) is from a CITES Appendix I animal; its trade is illegal in nearly every jurisdiction and it must not be obtained.** **Boy's urine (童尿) is a human material and pig bile (猪膽汁) a raw animal material; applying either to an open sore is a direct route for infection.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
