Donguibogam 東醫寶鑑

治蓄血證, 脉沈細微, 膚冷臍下滿, 或狂或躁, 大便色黑, 小便自利, 老幼氣弱者尤宜. 生地黃汁 一升<span class='sochu'>無則用生乾地黃 二兩</span>, 乾漆(炒烟盡) 半兩, 生藕汁 半升<span class='sochu'>無則用刺薊汁 一升半</span>, 生藍葉 一握<span class='sochu'>剉. 無則用乾末 半升</span>, 䖟蟲 二十箇(炒), 水蛭 十箇(炒), 大黃 一兩(剉), 桃仁(硏) 半兩. 右水三升, 同熬至二升, 放冷分二服, 先服至半日許, 血未下再服之. 此藥比抵當湯丸甚輕. 恐抵當丸下血不止, 故以此湯主之. 《海藏》

Translation

Treats the amassed blood pattern with the pulse sunken, fine and faint, the skin cold and fullness below the navel, mania or agitation, the stool black and the urine passing freely; it is especially suited to the old, the young and the weak. Fresh rehmannia juice 1 seung <span class='sochu'>or 2 nyang of dried fresh rehmannia if there is none</span>; dried lacquer, stir-fried until the smoke is gone, half a nyang; fresh lotus root juice half a seung <span class='sochu'>or 1.5 seung of thistle juice if there is none</span>; fresh indigo leaf a handful <span class='sochu'>cut up; or half a seung of the dried powder if there is none</span>; tabanus twenty, stir-fried; leech ten, stir-fried; rhubarb 1 nyang, cut up; peach kernel, ground, half a nyang. Reduce together in 3 seung of water to 2 seung, let cool and divide into two doses; take the first and wait about half a day, and if the blood has not come down take again. This medicine is much lighter than the Resist Decoction and Pill; fearing that the Resist Pill would bring down blood without stopping, this decoction is used to govern instead. — Haejang 海藏 (Donguibogam · Hyeol)

Clinical commentary 臨床

Zhongjing's Jidang-tang made safe — the same tabanus, leech, rhubarb and peach kernel, with three cooling juices added and the doses cut. The Haejang says why: the original brings blood down and does not stop, and this one is for patients who could not survive that. The presentation is the Shanghan tradition's amassed blood — black stool, free urine, mania or agitation, a cold skin over a full lower belly. The chapter's 1.10 treats it at length. **Contains tabanus, leech and dried lacquer, all toxic.
