Clinical commentary 臨床
One drug and a flour paste. The thinking is transparent — the coldest injury gets the hottest drug, and aconite unprocessed is the hottest thing in the book — and it is also why this is the most dangerous of the four frost-sore formulas.
凍瘡 at 2.14 covers two injuries that want opposite handling. Chilblains are an inflammatory response to repeated non-freezing cold and are mostly a nuisance; true frostbite is tissue that has frozen. Frostbite wants rapid rewarming in water near body temperature, 37 to 39 degrees, no rubbing of the frozen part and no dry heat, and no surgical decision for weeks, until the line between living and dead tissue declares itself. 爛痛, broken down and painful, is the stage at which this formula is given, and at that stage what the limb needs is clean dressing and time, not a counter-irritant.
**Raw aconite is severely toxic and this must not be prepared.** Aconitine is absorbed through broken skin, it is cardiotoxic, and there is no safe topical dose; applied to the ulcerated surface this formula is written for, the absorbing area is at its largest. Deaths from topical raw aconite are on record. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
