治凍耳成瘡. 黃栢ㆍ白斂 各五錢. 右爲末, 鹽湯洗後, 油調付之. 《得效》

Translation

Treats the frost-bitten ear that has broken into a sore. Phellodendron and ampelopsis root, 5 don each. Powder them, wash with brine, and apply stirred with oil. — Deukhyo 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

Two bitter astringents in equal measure, applied in oil after a brine wash. This is the mildest of the four frost-sore formulas and the only one that is harmless: phellodendron and ampelopsis root both carry tannins, which dry a broken surface and have some antibacterial action, and oil keeps the dressing from sticking. The ear is a classic site. Thin skin over cartilage with little underlying tissue and a poor blood supply makes the auricle one of the first places to suffer cold injury, and the chapter has a second remedy for it at 2.14 using powdered fritillary. The cases this is written for are chilblains rather than frostbite, and the sore stage is where a bland protective dressing genuinely helps. The one correction worth making is to the wash. 凍瘡 at 2.14 runs chilblains and true frostbite together, and they need opposite handling: a frozen ear must be rewarmed in water near body temperature with no rubbing and no dry heat, and no decision about dead cartilage should be taken for weeks. A brine wash on a chilblain is fine; on a frozen ear it is a distraction from the only thing that matters in the first hour. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
