Translation

Treats fright wind with recurrent convulsion, a hot body and stuporous sleep; it is able to bring phlegm heat down, and is a medicine for the Heart, Lung and Liver. Arisaema processed in ox bile, 1 nyang (if no bile-processed arisaema is to hand, take only the raw root, cut it up and fry it until done); bamboo sugar 5 don; realgar and cinnabar, 2 don 5 fun each; musk 1 don. Powder them, bind with licorice boiled to a paste into pills the size of a soap-bean seed — one text says the size of a gorgon-nut seed — and dissolve them down in warm water. For a child under a hundred days, one pill divided into three doses; for a child of five, one or two pills a dose. Boiling the licorice in last winter's snow water to bind the drugs is better still. 《錢乙》 ○ 抱 means to protect, and 龍 means the Liver. The Liver is the mother and the Heart is the child; when the mother is at peace the child is at peace. And since the Heart stores the spirit and the Liver stores the soul, once spirit and soul are settled, where should fright come from? — Jeoneul 錢乙 · Dansim 丹心 (Donguibogam · Soa)

Clinical commentary 臨床

The best-known paediatric formula in the tradition, and it is five drugs: bile-processed arisaema 1 nyang, bamboo sugar 5 don, realgar and cinnabar 2 don 5 fun each, musk 1 don, bound with licorice boiled to a paste. Qian Yi's own prescription, and the one leaf in 急驚風 that grades the dose by age — one pill split into three for a baby under a hundred days, one or two for a five-year-old. That is the closest the whole chapter comes to paediatric dosing, and it is still only a pill count. **Contains arisaema (天南星). Raw arisaema is a severe mucosal irritant — the calcium oxalate in it causes immediate burning, swelling and salivation, and in a small child a swollen airway is the danger — and it is safe to handle only after proper processing.** The ox-bile preparation is exactly that processing, and the leaf is honest that a substitute of merely fried raw root may be used instead, which is the version to refuse. **It also contains cinnabar (mercuric sulfide) and realgar (arsenic sulfide), at 2 don 5 fun each in a pill given to an infant, and musk from a CITES Appendix I animal. This must not be prepared.** The 丹心 gloss at the foot is the formula's name explained: 抱 is 保, to protect, and 龍 is the Liver — settle the mother and the child settles. It is a tidy piece of five-phase reasoning and it is why the pill is called what it is. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
