Jade Fat Ointment

Okji-go

한글 옥지고  ·  Pinyin Yu Zhi Gao

玉脂膏

治同上 refers back to the 鵝掌癬 of the preceding Changi-san leaf, and is translated out rather than left as a cross-reference. 桕油, the oil of the Chinese tallow tree, has no entry in this corpus's herb list and is named here rather than entered as an ingredient; the leaf's own substitute 鵝脂, goose fat, is entered under 白鵝, the herb list's headword for the bird, at the same weight. The interlinear 無則鵝脂代之 is a substance gloss and is set in parentheses, following the corpus's treatment of 無則狗頭骨代之. The transcription writes 醫鍳; the corpus prints 醫鑑.

Translation

Treats the same as above — goose-palm tinea. Cow's milk, tallow-tree oil (goose fat in its place if there is none), sesame oil and beeswax, 1 nyang each; melt them down, then add white lead 2 don, calomel 1 don 5 fun and musk 5 fun. Powder these, stir them evenly into the melted base, and smear it on the tinea; warm it at the fire and rub, warm it and rub again — the effect is like magic. — Uigam 醫鑑 (Donguibogam · Jechang)

Clinical commentary 臨床

A fat ointment of a quite deliberate kind: equal weights of cow's milk, tallow-tree oil, sesame oil and beeswax melted together into a base, and the three actives stirred in while it is still liquid. The instruction at the end — smear it on, hold the palm to the fire, rub, warm again, rub again — is what makes it work as an ointment, since heat and friction drive a fatty vehicle into thickened, fissured palmar skin. On an occlusive-emollient reading the base alone would have helped the scaling and cracking a good deal. What is in the base is the problem. White lead and calomel are the two actives, and the surface being treated is the palms, rubbed hard and warmed to improve absorption. 鵝掌癬 is itself the sequel to calomel taken for syphilis, as the section says outright, so this is mercury applied to treat a mercury injury. **Contains calomel (mercurous chloride) and white lead (lead carbonate), both absorbed through broken and macerated skin, and the method deliberately maximises absorption by warming and rubbing. Lead and mercury poisoning are cumulative and neurotoxic. Musk is from a CITES-protected animal. This must not be prepared or applied.** Thickened, fissured palms respond to an emollient and to removing the cause; a plain petrolatum or urea ointment does the same work as this base, without the metals. — Eui Young (Justin) Chung, L.Ac., Dipl.
