Green Cloud Plaster

Nogun-go

한글 녹운고  ·  Pinyin Lü Yun Gao

綠雲膏

The leaf gives no 治 clause; the indication comes from the 外治法 passage at 033b, which has this plastered over a node pierced with a fire needle and packed with Seomsu-go. 栢 and 鼈 are left as the transcription prints them; the corpus does not normalize 栢 and 柏, and prints 鼈 far more often than 鼈. 香油 is registered under 淸油 as cheongyu. The three added drugs are conditional on 瘡口不乾 and are entered with that noted in the preparation. The water the salve is strained into and pulled in is part of the method, not an ingredient.

Translation

Coptis, rhubarb, scutellaria, scrophularia, phellodendron and the kernel of momordica, 1 don each. Cut them up and fry them in 1 nyang of sesame oil; when they are scorched in colour take out the dregs, put in 5 nyang of pine resin and boil again to a salve. Strain it into water and pull it out like taffy until it is the colour of gold, boil it down again, let it cool to warm, and put in the bile of three pig gallbladders and 3 don of verdigris that has been steeped in vinegar overnight with the dregs taken off. Stir it even and spread and plaster it in the usual way. If the mouth of the sore will not dry, add frankincense, myrrh and calomel, which is better still. — Jeongjeon 正傳 (Donguibogam · Jechang)

Clinical commentary 臨床

A classic oil-and-resin plaster with the whole bitter wall of the chapter fried into it — coptis, rhubarb, scutellaria, phellodendron, scrophularia and a momordica kernel, scorched in a nyang of sesame oil until the drugs give out and are discarded. Five nyang of pine resin then sets it, and the pulling of the warm mass in water until it turns gold is the standard way of working a plaster soft and even. The green of the name comes in at the end, with the verdigris. Verdigris and pig bile are the two active finishing drugs, and both are chosen for a sore that will not dry: copper acetate is an astringent antiseptic and bile is a surfactant that keeps a wound surface clean. The conditional addition is the giveaway as to what these nodes actually did — 瘡口不乾, the mouth of the sore will not dry, is a tuberculous sinus that has been draining for months. **Contains verdigris, a copper salt that is toxic and irritant, and a momordica kernel, which is toxic. The conditional addition is calomel, a mercury compound absorbed through broken skin. This must not be prepared.** A discharging neck sinus is tuberculous lymphadenitis until proved otherwise and needs tissue diagnosis and a full antimycobacterial course; an occlusive plaster over it does nothing but hold the discharge in. — Eui Young (Justin) Chung, L.Ac., Dipl.
