Donguibogam 東醫寶鑑

厚朴ㆍ白茯苓ㆍ澤瀉ㆍ半夏 各一錢, 蒼朮 八分, 靑皮ㆍ陳皮 各六分, 草豆蔲ㆍ人參ㆍ當歸 各五分, 益智仁ㆍ吳茱萸 各三分, 木香ㆍ乾生薑ㆍ升麻ㆍ柴胡ㆍ甘草 各四分. 右剉, 作一貼, 入薑三, 水煎服. 《寶鑑》 經曰, 留者行之, 結者散之. 以柴胡ㆍ升麻之苦平, 行少陽, 陽明二經, 發散淸氣, 運行陽分爲君. 以生薑ㆍ半夏ㆍ草豆蔲ㆍ益智仁之辛甘大熱, 消散中寒, 爲臣. 以厚朴ㆍ木香ㆍ蒼朮ㆍ靑皮之苦辛大溫, 通順滯氣, 當歸ㆍ人參ㆍ陳皮之辛甘溫, 調理榮衛, 滋養中氣, 氣之薄者, 陽中之陰, 茯苓ㆍ甘草ㆍ澤瀉之氣薄, 導引濁陰之氣, 自天而下故, 以爲佐. 濁氣不降, 以苦泄之, 吳茱萸苦熱, 泄之者也. 故以爲使. 氣味相合, 散之泄之, 上之下之, 使淸濁之氣, 各安其位也. 《寶鑑》

Translation

Magnolia bark, white poria, alisma and pinellia 1 don each; atractylodes lancea 8 pun; green tangerine peel and tangerine peel 6 pun each; tsaoko, ginseng and angelica 5 pun each; alpinia and evodia 3 pun each; costus, dried fresh ginger, cimicifuga, bupleurum and licorice 4 pun each. Cut up, make one packet, add 3 slices of ginger, decoct in water and take. The classic says: what lodges, move it; what binds, scatter it. Bupleurum and cimicifuga, bitter and level, travel the lesser yang and yang brightness channels, effuse and scatter the clear qi and set the yang aspect moving — these are chief. Fresh ginger, pinellia, tsaoko and alpinia, acrid and sweet and greatly hot, disperse and scatter cold in the centre — these are deputy. Magnolia bark, costus, atractylodes lancea and green tangerine peel, bitter and acrid and greatly warm, free and normalize stagnant qi; angelica, ginseng and tangerine peel, acrid and sweet and warm, regulate the construction and defence and enrich the centre qi. What is thin in qi is the yin within yang: poria, licorice and alisma, being thin in qi, conduct the turbid yin downward from heaven — these are assistant. Turbid qi that does not descend is drained with the bitter, and evodia, bitter and hot, is what drains it — hence it is envoy. The qi and flavours joining, scattering and draining, raising and lowering, the clear and turbid qi each come to rest in their proper place. — Bogam 寶鑑 (Donguibogam · Changman)

Clinical commentary 臨床

The fullest chief-deputy-assistant-envoy analysis in any of the chapters translated so far, and it repays reading as a model. Every medicinal is assigned a flavour, a temperature, a channel where relevant, and a role, and the whole is closed by naming the aim: the clear and turbid each come to rest in their proper place. Note that bupleurum and cimicifuga are chief. In a formula for distension, the medicinals that lead are two that lift — because on this reading the fault is direction, not substance. 濁氣在上 means the turbid has risen where the clear should be, and the remedy is to send each back. The case recorded alongside it at 3.7.2 makes the same point practically: a patient whose distension worsened in the depth of night was first moxaed at Jungwan to draw the stomach's engendering qi upward, and only then given this decoction. — Eui Young (Justin) Chung, L.Ac., Dipl.
