Translation

Treats visceral cold with a slippery diarrhoea, the grain passing untransformed, heat above and cold below, mouth sores and wasting. Costus root and stir-fried psoralea, 1 nyang each; lesser galangal, amomum and magnolia bark, 7.5 don each; red peony, tangerine peel, cassia bark and white atractylodes, 5 don each; evodia and black pepper, 2.5 don each; nutmeg roasted in dough, four fruits; areca, one fruit. Powder; for each 3 don take 4 nyang of pig's liver, split it open, layer the powder through it, put it in a bowl of rice-water with a little vinegar, cover and cook it through, add salt, three stalks of scallion white and ginger the size of a pellet, and cook together until the water is nearly gone. Take it as one dose on an empty stomach, eaten cold. A slight looseness at first does no harm. For a slippery diarrhoea or a cold dysentery of years' standing, one dose is effective. If thirsty, drink gruel water. — Seui Deukhyobang 得效 (Donguibogam · Daebyeon)

Clinical commentary 臨床

The most elaborate preparation in the chapter and the most interesting indication: heat above with cold below, mouth ulcers in a wasting patient with chronic diarrhoea. That constellation is real — aphthous ulceration with malabsorption is how coeliac disease and Crohn's often present — and the tradition reads it correctly as one disorder rather than two. The powder is cooked into pig's liver and eaten cold as a single meal, which makes it a food-medicine; given that the patient is wasting and probably anaemic, four nyang of liver may have been the most therapeutic part of it. — Eui Young (Justin) Chung, L.Ac., Dipl.
