Translation

Treats lung abscess with panting and inability to lie down; it also treats propping fluid with urgent panting. Lepidium seed, dry-fried to a yellow and powdered, bound with honey into a pill the size of a bullet; take twenty jujubes and three seung of water, decoct down to two seung, remove the jujubes, put in one pill, decoct again down to one seung, and give it all at once. ○ Son Jo treated a man who in a short while had brought up a seung of phlegm and whose panting and coughing would not stop; after one dose of this he felt the chest clear and easy and had almost no phlegm or spittle left. — Uihak Gangmok 綱目 (Donguibogam · Haesu)

Clinical commentary 臨床

Junggyeong's formula, with the case report the Gangmok attached to it. 肺癰 is a suppurating lung — an abscess or an empyema — and 支飮 is fluid propped in the chest, which is a pleural effusion; the formula drains both by the gut. The case describes a man bringing up a pint of sputum who was relieved by one dose, which is what a drained empyema looks like from outside. 喘不得卧, panting that will not let the patient lie down, is orthopnoea, and the same sign at the head of Sinbi-tang below. **A patient with copious purulent sputum and orthopnoea needs imaging and antibiotics, and often drainage. Lepidium seed is a strong diuretic and purgative.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
