Donguibogam 東醫寶鑑

治上氣喘嗽浮腫. 紫蘇葉二錢, 紫菀ㆍ甘草各一錢, 陳皮ㆍ桔梗ㆍ麻黃ㆍ桑白皮ㆍ阿膠珠各七分半, 五味子ㆍ大腹皮ㆍ烏梅肉ㆍ杏仁各五分. 右剉, 作一貼, 入薑五片, 水煎服. 《丹心》

Translation

Treats qi rising with panting cough and puffy swelling. Perilla leaf, 2 don; aster and licorice, 1 don each; tangerine peel, platycodon, ephedra, mulberry root bark and ass-hide glue rolled to pearls, 7 fun 5 ri each; schisandra, areca husk, smoked plum flesh and apricot kernel, 5 fun each. Cut up, make one packet, add five slices of ginger and decoct in water. — Dangye Simbeop 丹心 (Donguibogam · Haesu)

Clinical commentary 臨床

浮腫 — puffy swelling — alongside the panting is what this formula is for, and it is the combination the chapter calls 水喘: wheeze with oedema. The ephedra and perilla leaf open the surface, mulberry root bark and areca husk move the water, and the glue and smoked plum keep a drying formula from stripping a patient who is already short of fluid in the blood while waterlogged in the tissue. Wheeze with swelling of the legs and face is cor pulmonale or heart failure to a modern examiner, and the chapter's own section on it reasons in the same direction: 喘必生脹, 脹必生喘 — panting breeds distension and distension breeds panting. **Contains ephedra, which is a sympathomimetic and raises blood pressure and heart rate; in a patient with heart failure it is dangerous and must not be used.** — Eui Young (Justin) Chung, L.Ac., Dipl.
