Donguibogam 東醫寶鑑

治肺氣喘促, 面目浮腫. 葶藶子ㆍ防己ㆍ木通ㆍ杏仁ㆍ貝母各一兩. 右爲末, 蒸棗取肉和丸梧子大. 桑白皮湯下五十丸. 《入門》

Translation

Treats lung qi with hasty panting and oedema of the face and eyes. Lepidium seed, stephania, akebia, apricot kernel and fritillaria 1 nyang each. Powder them, take the flesh of steamed jujubes and make pills the size of a firmiana seed; take fifty down with a decoction of mulberry root bark. — Uihak Immun 入門 (Donguibogam · Bujong)

Clinical commentary 臨床

Five medicinals at equal weight, all aimed at the lung. Lepidium seed drains water from the lung forcefully and is the classical medicinal for the case where fluid has reached the chest; steamed jujube flesh as the binder and mulberry root bark as the vehicle both moderate it. Swelling that begins at the face and eyes is the chapter's marker for water in the upper burner, which is why this formula and the two ephedra decoctions share that indication. **Facial oedema with breathlessness requires assessment — nephrotic syndrome and cardiac failure both present this way.** — Eui Young (Justin) Chung, L.Ac., Dipl.
