Donguibogam 東醫寶鑑

治蜘蛛蠱脹. 肉蓯蓉 三兩, 靑礬ㆍ紅棗ㆍ香附子 各一斤, 麥芽 一斤半. 幷爲末, 先將蓯蓉ㆍ棗, 礬入罐內, 同火煆烟盡, 和藥末, 糊丸梧子大. 每二三十丸, 食後酒下. 勞, 單腹腫大, 而四肢極瘦者, 名曰蜘蛛蠱. 古方雖有八物湯<span class='sochu'>方見虛勞</span>, 去地黃, 倍參朮, 加黃連ㆍ厚朴, 及諸蠱保命丹ㆍ蝦蟆煮肚法<span class='sochu'>見上浮腫</span>, 然此皆脾氣虛極, 眞藏已傷病也, 不治. 《入門》

Translation

Treats spider gu distension. Cistanche 3 nyang; green vitriol, red jujube and cyperus 1 geun each; barley sprout 1.5 geun. Powder them all; first put the cistanche, jujube and vitriol into a jar and calcine together until the smoke is spent, then mix in the remaining powders and make paste pills the size of a firmiana seed. Take twenty or thirty after meals with wine. In taxation, where the belly alone swells large while the four limbs are extremely thin, it is called spider gu. Although the old formularies give Eight Substances Decoction <span class='sochu'>formula in the Vacuity Taxation chapter</span> with rehmannia removed, ginseng and atractylodes doubled, and coptis and magnolia bark added, together with this All Gu Preserve Life Elixir and the method of boiling a toad in a pig's stomach <span class='sochu'>see the Oedema chapter above</span> — yet all of these are cases in which the spleen qi is utterly vacuous and the true viscus already damaged, and they cannot be treated.

Clinical commentary 臨床

蜘蛛蠱, spider gu — a swollen belly on wasted limbs, the shape of a spider. It is the chapter's name for the end stage, and the passage that closes the entry is unusual for listing three treatments and then saying that none of them works. That honesty is the point. The spleen qi is utterly vacuous and the true viscus already damaged; the formulas are recorded because they were used, not because they were expected to succeed. The section on prognosis at 3.9 takes the same position and gives it a name — distension is a true-viscus disease. The clinical picture is decompensated cirrhosis with ascites and muscle wasting, and the judgement recorded here matches what it was before modern care. **Contains green vitriol, an iron sulfate preparation, in large quantity.
