Donguibogam 東醫寶鑑

治肺痿咳唾膿血, 欲變成癕. 紫菀ㆍ知母ㆍ貝母各一錢半, 人參ㆍ桔梗ㆍ赤茯苓各一錢, 阿膠珠ㆍ甘草各五分, 五味子三十粒. 右剉, 作一貼, 薑三, 水煎服. 《入門》

Translation

Treats wilting lung with the coughing and spitting of pus and blood, about to turn into an abscess. Aster, anemarrhena and fritillaria, 1 don 5 fun each; ginseng, platycodon and red poria, 1 don each; ass-hide glue roasted to beads and licorice, 5 fun each; schisandra, thirty grains. Cut up, make one packet, add three slices of ginger, decoct in water and take. — Uihak Immun 入門 (Donguibogam · Haesu)

Clinical commentary 臨床

Platycodon with aster and fritillaria is the tradition's combination for expectorating pus, and the glue and schisandra are there for the blood in the sputum. 欲變成癕 — about to turn into an abscess — names the transition the chapter is watching for, and 肺痿 2.7's distinction between the two is drawn on exactly this point: frothy sputum is 肺痿, while a dry rattling cough with chest pain and a slippery rapid pulse is 肺癕, the abscess. **Coughing pus and blood needs sputum culture and chest imaging; a lung abscess requires antibiotic treatment.** — Eui Young (Justin) Chung, L.Ac., Dipl.
